Required First Visit for Services - Registered Nurse Start of Care
Comprehensive intake with health history, vitals, medication management, education and reconciliation, home-safety/fall risk check, and creation of an initial care plan with provider coordination. Transparent price of services will be disclosed at this time.
Behavioral Health Support / Case Management On-going Nursing Visits
Symptom screening, coping/relapse-prevention coaching, linkage to resources, and care coordination with documentation of progress. Includes post-procedural monitoring, wound care, and other skilled nursing tasks.
Non-Clinical Protective Companion
Provides supervision and companionship to promote safety, redirection, and social engagement; cues for hydration, mobility, and orientation.
After-Hours / Emergency Nursing Visit
Urgent, time-sensitive nursing support outside standard hours (triage, focused assessment, medication issues, wound concerns); subject to availability.
Safety Attendant with ADL Assistance
Certified Caregiver provides supervision and companionship to promote safety, redirection, and social engagement; offers cues for hydration, mobility, and orientation; and minimal assistance with activities of daily living (e.g., dressing, toileting, transfers).
Heavy Home Health Aide / Personal Care / House Chores & Non-structural Maintenance
More labor-intensive tasks (e.g., deep bathroom/kitchen clean, multiple laundry loads, organizing, moving light furniture, minor non-structural fixes). No licensed trades or hazardous work.
Nurse Caretaker
Nursing care that combines Protective Companion/Safety Attendant support with a clinical assessment for medication administration.
*Required if assessing the patient and administering as needed medication of any kind.
Light Home Health Aide / Personal Care Services
Routine personal care and household assistance (bathing, hygiene, grooming, meal prep, light housekeeping, linens, reminders).
After-Hours / Emergency Safety Attendant / Aide Visit
Urgent accompaniment or ADL/household support outside standard hours; “light” for routine help, “heavy” for more labor-intensive needs; subject to availability.
